- List of all medications that you are currently taking.
- Driver’s license.
- Insurance and prescription cards.
- If applying for financial assistance, please bring your most recent tax return, a recent bank statement, and any Social Security letters, if applicable.
At each subsequent visit, please let us know of any changes your insurance, contact information, marital status, or employment.
